The Federal Trade Commission (FTC) and the New York State Department of Health both require that certain disclosures be made by funeral providers on the General Price List. Although these disclosures may appear to be similar, they are not exactly the same. This General Price List contains both sets of the required disclosures. The disclosures required by the FTC, when different from those required by the Department of Health, are preceded by the letters "NYS."

(FTC, NYS) The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. [“However, any funeral arrangements you select will include a charge for our services” (NYS).] However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you
must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

(FTC, NYS) This list does not include prices for certain items that you may ask us to buy for you, such as cemetery or crematory services, flowers, and newspaper notices. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected.
